Document education in inpatient medical record within 24 hours of service. Provide documentation to physician offices within one week for outpatient referrals.? Complete nutrition assessments on patients at nutritional risk at SJRMC and SJRRH. Make recommendations for medical nutrition therapy: Complete assessments within timeframe as per policy. Notify physician and other professional staff of concerns/recommendations. Enter assessment into medical record.? Follow-up evaluation for nutrition risk patients: Re-evaluate and review care plan within at least 1 week of initialassessment and/or prn. Document.? Respond to telephone inquiries re: nutrition: Respond to phone inquiries re: nutrition. If by phone message, attempt return call within 48 hours of receipt of call.? Conduct nutrition education classes for patients and in-house staff. Examples: cardiac, pulmonary and diabetes classes for patients; graduate nursing students and physician CME: Design and present classes with most current nutrition information.
